APPLICATIONS WILL ONLY BE PROCEED AFTER RENEWAL OF MEMBERSHIP.

Up to three tests can be submitted on this form. Forms maybe submitted between June 1, 2024 and July 15, 2024. No applications accepted after July 15, 2024, NO EXCEPTIONS.
ONLY Tests taken at an Oregon Skating Council Test Session are eligible for reimbursement.

You may apply for reimbursement for up to three tests with this form. You may submit additional applications if needed. The CSFSC Test Reimbursement Fund only guarantees reimbursement of 50% of two tests passed per year. If more funds are available a higher amount may be awarded.
